Overview
Osteoarthritis (OA) is a debilitating degenerative disease that affects synovial joints in the body, with the knee being one of the most commonly affected joint. OA has no known treatment or cure primarily because the spatio-temporal degradation patterns of tissues involved in knee OA are unknown. The most commonly used imaging-based biomarkers of OA progression are joint space width and cartilage thickness loss, but these only depict gross morphological changes in advanced disease states. For the past two decades, quantitative MRI methods have been investigated extensively as potential biomarkers of early degeneration of soft tissues affected by OA. Despite the excitement, such quantitative metrics have seen limited adoption in large scale drug trials for OA or for routine use in diagnostic clinical imaging and orthopedic surgery. In this symposium, we will provide an overview of some of the most promising quantitative MRI measures and critically discuss the challenges that have been faced in pervasive implementations. Additional talks will cover specific application areas such as large-scale population studies, diagnosing early OA degeneration in routine clinical scans, assessing outcomes in orthopedic surgery, and evaluating efficacy in drug trials. All four areas have the potential to contribute to our understanding of OA, especially in instances where quantitative imaging may play a beneficial diagnostic or prognostic role.

ISMRM members working on quantitative MRI MSK methods and clinicians and scientists working to improve treatments in degenerative joint disease.

- Identify emerging quantitative MRI methods for imaging tissue microstructure for early detection of joint degradation;
- Explain challenges that have led to limited clinical translation of these methods; and
- Discuss opportunities and needs to incorporate new quantitative methods in population studies to understand disease pathogenesis for development of new disease-modifying therapies, and in clinical radiology and orthopedic surgery.
